2013年1月17日 · Do not give NS with bicarb. ODS is no bueno for happy neurons. You are essentially giving hot salt. D5 with 3 amps or 1/2NS with 2 amps if there CVP is in the toilet and you need volume in addition to correction of acidosis (the later is around 177 Meq, not a ton more than isotonic NS)

Pearl: if you are planning to give more than 3-4 liters of normal saline, switch to plasmalyte or lactated ringer's (LR) because of the risk of "expansion acidosis". These fluids are isotonic, just like normal saline. Hypervolemic patient: avoid fluids at all costs.

2009年7月5日 · Once they are no longer tachycardic and are making urine (most sensitive sign of euvolemia) then you switch to half-normal saline in adults and 1/4 normal saline in children. Remember that you probably need to add 20 mEQ of K and probably Mg because you're giving IV fluids, as well as D5 if the patient is NPO.

2009年7月9日 · Actually, it's not the veins but those pesky little red cells that don't like to be exposed to hypotonic solutions. A 5% dextrose or a normal saline solution is hypertonic to blood plasma. The most common solution that I use for giving "water" is D5 and 1/2 NS which is kind to erythrocytes and feeds the brain at the same time.

2007年1月2日 · The one half normal saline is likely given with the dextrose to prevent central pontine demyelination so that the hypernatremia will not be corrected too quickly. Ideally, you should figure otu a total water deficit with hypernatremia to figure out how much to give, but in the setting of DKA, I don't think that those equations apply anymore.

But under-resuscitation is a much worse crime than over-resuscitation IMO, and if you're paying attention to the fluid responsiveness of your patient, continuing the fluids is almost certainly more helpful than not...

2009年2月9日 · What he means is the dextrose in the IVF doesn't stay intravascular. It crosses from the plasma to cells in the body. As far as tonicity, how many meq of sodium and chloride in 0.9NS? 154 meq of each for a total of 308. 0.45NS has 77 of each for a total of 154meq. I remember 300meq for a normal blood osmolality.

2008年8月21日 · The software at my clinical site doesn't let me choose normal saline as an option for a compatibility check. The only thing I can check is if the bag is compatible with D5W or as is compatible w/ electrolytes like potassium chloride or something else.
