Just Photoshop. Found a topographical map of California and Nevada, changed the hue of it just a bit. Found a Zip File with Fallout Map Markers. Spend a few hours in front of my computer screen with Photoshop and Fallout Wiki open at the same time. It was only after I was working on it that I saw programs for making Fallout maps.

Apart from the official one, here is bunch of alternatives listed: https://www.reddit.com/r/rpg/wiki/fallout. The official Licensed Fallout 2d20 by modiphius is something of a clusterfuck.
