My calculated HOMA-IR score is then (12.9*124)/405 = 3.95. A score of 1.0 or less is normal from what I've read -- i.e. not insulin resistant.

I found a chart at this link that shows the progression to diabetes and it shows an increase gradually in fasting insulin over time, with the fasting insulin starting at 12. Thanks guys.
