Medical records must be submitted using Bucknell's official record form. The form includes a medical provider packet that must be completed by a physician within twelve months of the start of classes.

Alternatively, students can upload any records to the MyCUHealth portal. Please also complete the Medical History form online through the MyCUHealth portal.
